IMAGE CONSENT AND RELEASE FORM
By agreeing to this release form, I confirm I am 18 years or older in age and agree that Foundever®, Foundever Operating Corporation and other entities within the group of companies may:
1. Photograph, record video and/or audio or otherwise reproduce my likeness (further referred to as “My Recording”).
2. Receive photographs, videos or audio recordings provided by me (further referred to as ‘Provided Recording’).
3. Store and retain copies of My Recording and/or Provided Recording.
4. Use My Recording and/or Provided Recording in connection with company and promotional activities of Foundever such as, but not limited to, business presentations, internal communications, printed publications and sales & marketing collateral.
5. Edit, cut, shorten and rearrange the My Recording and/or Provided Recording as it deems appropriate without the need for me to review or approve the altered result. I waive the right to any royalties or compensation, now and in perpetuity, arising from or relating to, the use of My Recording and/or Provided Recording by Foundever. To the extent possible, I waive all moral rights associated with My Recording and/or Provided Recording.
I represent and warrant that the Provided Recording does not include any third-party intellectual property rights, is provided free and clear of all encumbrances and is provided on a royalty free, perpetual basis to use, distribute, reproduce, sell, publish and re-publish and/or create derivative works. I further acknowledge and agree that Foundever will own fully any intellectual property rights created through its use or derivative works.
I fully understand that my consent is freely given and that I can withdraw it at any point by a written request to privacy@foundever.com. I understand the withdrawal of my consent will only have future effects on the processing of my personal data. I understand that I have the right to at any point ask for details regarding the processing of my personal data and exercise my rights under the data protection law.
